Blood Stream Infection (BSI) Checklist

Problem Statement:   Catheter-related blood stream infection is associated with increased morbidity, mortality and costs of care.  Indeed, this preventable complication causes 11 deaths each day in U.S. hospitals.  ICU patients are particularly at risk because 48% have indwelling central venous catheters that account for 15 million central catheter days per day in the U.S.

Purpose of Tool:  To improve teamwork and focus efforts on complying with infection control practices for central venous and central arterial line insertions or rewires.  The ultimate goal is to eliminate patient harm from catheter-related blood stream infections.

Who Should Use:   Bedside nurse

How to Use:  This checklist should be completed by the bedside nurse while the physician is preparing for a central line insertion.  If the team member prepping for the line insertion fails to complete a step and jeopardizes a sterile field, the nurse will stop the process.  The team will regroup and start the process over again.  Checklists are turned in to a designated location assigned by the unit leader.
